Your EHR is the system of record and it is good at that. It can flag a patient as due, and it can hold the order once somebody places it. What it does not do is carry the work between those two points: finding everyone who qualifies today, sending the outreach, and following the referral until the result comes back. That stretch in the middle is where care goes missing, and it is the only thing MedFlow does. Nothing in your EHR is replaced and there is no second system to open.

Usually alongside them rather than instead of them. A partner brings a program: the measures, the clinical model, sometimes the funding.
